How to Get Started With In-Home Care

Getting started with our in-home caregiver services is easy. The first step in the process to getting an in-home caregiver for your senior loved one begins with filling out the form below and submitting it to us. Once your inquiry is received, our care coordinator will contact you promptly.

 

In-Home Assessment

Second, is scheduling the in-home assessment to evaluate the sensitive needs of your senior family member. During this visit, our compassionate and knowledgeable registered nurse will work closely with you and your family to review your current health status, living environment, and care needs. While walking you through the process of getting home care, the RN's assessment will focus on four care areas that are critical to the health, safety and well-being of your senior loved one: cognitive stimulation, socialization, physical health, and safety supervision. Also, the in-home assessment is when you will have the option to meet and greet with your caregiver who may accompany your RN during this visit. This is your chance to ask any questions that you may have of your caregiver.

Personalized Plan of Care

 

The next step is carefully curating a personalized Plan of Care and care schedule. Based on the RN's in-home assessment, we will develop a comprehensive Plan of Care that outlines the specific services and support you require, as well as the schedule and frequency of visits. We understand that there is no one size fits all care plan and consider factors such as activity level and personal interests.

 

Caregiver Matching

 

Our care team carefully matches a caregiver and provide her or him with specific detailed of the responsibilities she or he will have. Caregivers 24/7 understands the importance of finding the perfect caregiver match. We take into account characteristics such as personality and common interests to ensure a comfortable and compatible fit. The process from initial inquiry to the first day of home care typically takes no more than 24 hours. 

Ongoing Case Management

 

Lastly, is on-going monitoring, supervision, and communication by our RN to ensure the highest standard of care and service. Your care plan will be regularly reviewed and updated by our care team to ensure that your changing needs are met. We will regularly follow up with you and your family throughout the process to address concerns or answer any questions that may arise.  The comfort and happiness of our client and their family are our measure of success.
